1) OFFLINE ( with required documents )

1. Fill in tmnet streamyx eCosway Application Form available at all Cosway Sales Centres and TM Net State Representative Centers or please click on the links below to download the form:
   - Page 1
   - Page 2
   - Page 3
   - Page 4

2. Fax the form with the required documents directly to TM Net Channel Development & Management Unit at 03-7806 3144, or submit to any TM Net State Representative nationwide.
   
   Note : Required documents:

   *Streamyx Basic	- photocopy of IC (for Malaysians) / Passport (for non Malaysians).
   *Streamyx SOHO, Enterprise and Corporate	- photocopy of Forms D, B or A, Forms 9, 49 and Forms 24 or 23 (whichever applicable).
